Letter of Recommendation Guidlines:

You must print and fill out a FERPA form and submit it to me as required by the University of Utah.

I will only write a letter for you if the following boxes on the FERPA form are checked:
1. Grades
2. Class Rank
3. Courses Attended
4. Academic Performance
5. I waive my right to review a copy of the letter at any time in the future.

Additional Requirements:

1. You must give me 3-4 weeks to complete the letter.
2. I must be able to submit the letter electronically.
